St. Francis Home provides care to those who require short-term stays as a result of medical or surgical conditions that require treatment for a limited period of time. These services may include restorative care and/or physical, occupational, speech or respiratory therapy.

St. Francis Home has experienced nurses that have consistent assignments and provide primary care nursing to patients on the Skilled Nursing and Rehab Unit. Consistent assignment is a key step in giving care that is centered on the patient. Patients receive better care because the nurses are more knowledgeable about their individual strengths, needs and preferences. The nurses are better able to implement care plans and are more likely to notice changes in a patient’s health, which can prevent larger health problems. Patients often feel more secure and comfortable knowing that they will receive care from someone who know and understand their personal goals.

Short-term rehab services include comprehensive physical, occupational and speech therapy. St. Francis Home, Inc. has teamed up with HealthPRO Heritage, who provides individualized treatment plans that enable people to return to their home.
